An 81-year-old male with a joint balance of 0/2/70° six months after total knee arthroplasty in whom a diagnosis of arthrofibrosis was confirmed after ruling out other aetiologies and performing an arthroscopic arthrolysis. Due to the presence of the femoral prosthesis, we observe this mirror image of the adherences found in the pouch zone. The adherences were freed from the pouch and medial and lateral grooves, achieving an intraoperative joint balance of 0/0/110°.
